License Renewal - Physicians
Physician licenses can be renewed using the online renewal form. Please login to access the online renewal form.
- Online renewal fee is $ 400.00
- Processing fee is 2.25% of total fee
Best Practice Prescribing of Controlled Substances and Drug Diversion Training Verification form must be submitted with your license renewal fee and CME hours attached, to the Board, by June 30th. If submitted after June 30th, you must include a $100.00 late fee. If you fail to renew within 90 days, you must complete a new application and pay a penalty, as prescribed by law, not to exceed $100.00.
If you will attest you HAVE NOT prescribed, administered, or dispensed a controlled substance during the last renewal period, please take a moment to download the CME Waiver Request form.
While you are in the renewal process please do not use the back button in your browser. If you will be away from your computer for any period of time after starting your online renewal, it is strongly advised that you click on the "Save for Later" button located at the bottom of each page to minimize the risk of losing the data that you have input into the system. You can then log back into the renewal at a later time by logging into the online renewal. If at any time you wish to exit the renewal process, please click the "Cancel & Exit" button located at the bottom of the page. Please note if you select "Cancel & Exit" you will lose any data entered.
Prior to your license being approved, please pay the $127.82 Patient Care Injury Fund Assessment. This new assessment was approved by the 2016 West Virginia State Legislature and will be required for each license issued from now until December 31, 2021. NONE of this money will be deposited in the WV Board of Osteopathic Medicine's Fund. This is a state-mandated assessment.
